[MLB-WIRELESS] [TIB] Where to send Money??

When completing the online form to express an interest in
a bulk purchase of an Alcatel modem, I expected to have to enter
credit card or other payment details... it didn't reach that point.

referring to:

The offer closes today.
It also says payment must be made by today.
Does anyone know where to send the required payment?
or perhaps, who is looking after the deal?
